Control Number:

This field  is a number or code provided by the requestor to help the requestor and the person or team who is servicing the request to track the progress of the help request.  It is the requestor's responsibility to provide a control number.  Although it is not important What the number is, it is very helpful to have one. Requests that are received without a Control Number may be assigned a default number of "9999".

E-Mail Address:
This field is used to send a confirming e-mail notification to the e-mail address provided.  The address format can be either of the following forms:

   First.Last@UToledo.EDU  (ex: Lowell.Gill@UToledo.EDU)                                                                           

   name@utnet.utoledo.edu   (ex: lgill@utnet.utoledo.edu)

Please be aware that from time to time, it is possible that the e-mail server will be dysfunctional and your confirming e-mail will not be delivered.  

Scope:
This field gives an indication of the scale of people, departments, or computer systems are affected by the requested work.  For example, if only the requestor's computer is not working, then the scope is low, but if all the servers for the University are affected, then the scope could be high. This value is tentative and subject to modification by the appropriate system management team.  This field is used to assess work priority.

Impact:
This field indicates the severity of the problem.  If the problem is mainly a nuisance then the value should be low. However, if the problem affects the requestor's or the department's productivity, then the value should be higher.

Mandate:
This field gives an indication of the reason why the originator created the help request and helps the system's management team or work group to rate the relative importance, or priority, of this work in comparison to all other outstanding work that has been previously assigned to the team or group.   This field is used to assess work priority.

Complete Work By This Date:
This field gives the system's management team or work group a target date by which the requested work should be completed and available in a production environment.  Bear in mind that this help request will compete with work currently in progress and with help requests received previously.  This is a tentative date, and is subject to review and revision by management teams or work groups.
