  • MS SQL Database Access Request - Use the form to request access to SQL tables within a database. Database access is required to query the tables and use the MS Access Report Libraries.

  • MS Access Report Library Move Request - Use the form to request moving Access objects (tables, queries, forms, reports, etc.) to the pertinent MS Access Sungard Plus Report Libraries (SIS, FRS, or SAM).

  • Database Request for Applications Development - Use the form to request access to a database for application development.

  • Oracle Object Move Request - Use the form to request moving Oracle database objects from a test to a production database.  See the Oracle Object Move Instructions for details.

  • Oracle Script Execution Request - Use the form to request an Oracle Script be executed.

  • SQL Object Move Request - Use the form to request moving SQL Server Data Warehouse objects (tables, views, stored procedures, user defined functions, DTS, etc.) from the test to the production database.

  • IAS Report Developer Request - Use the form to request access to the Institutional Analysis System (IAS) tables.  The IAS contains student oriented snap-shot information for analytical reporting and analysis.  Users must possess a knowledge of Crystal Reports and have attended a training session as prerequisite for access.  To schedule training, please contact Daryl Wright in Institutional Research.

  • Lawson Material Mgmt Request - Use the form to request access to the Lawson Materials Management or Accounts Payable areas, or to request access to the database for querying.
